• Matriculation or its equivalent certificate to give the proof of date of birth.
• Mark Sheets of the qualifying examination and all the previous examinations such as High School, Intermediate, Graduation, Post – Graduation.
• Medical Certificate issued by the appropriate authority, if admission is taken under Physically Challenged Category.
