• For M.Sc., MBA, MPH courses, candidates have to fill the online application form given on the official website of the university along with Information Brochure. Send the print out of the form with the required documents to the given address below.

• The envelope must be superscripted with “M.Sc./MBA/MPH (N) – Application Form”.
• For Diploma Admission, the envelope must be superscripted with “Application form for Diploma in Ophthalmic Science/ Diploma in Optometry 2025-25”.

• Nationality Certificate/ Proof issued by the Competent Authority.
• Domicile Certificate or Birth Certificate.
• SSC & HSC Certificate.
• Photocopy of Valid Passport duly attested by Head of Department.
• CET 2025 Marksheet & Online Allotment letter of Respective Round (If Applicable).
• Passing /Degree Certificate of the qualifying examination.
• Internship Completion Certificate (If applicable).
• Valid Registration certificate from the Respective Council or attach renewal receipt.
• Caste Certificate (If Applicable).
• Caste Validity Certificate (If applicable).
• Non – Creamy Layer Certificate valid up to March 31, 2025.
• College leaving Certificate (LC/TC).
• Attempt Certificate of all examinations in Degree course from Head of the Institute (If applicable).
• Gazette for change in name (If applicable).
• Migration Certificate issued by the respective University (If applicable).
• Self-Educational Gap Certificate (after qualifying Degree) (If applicable).
• Medical Fitness Certificate as per brochure format.
• Physically Handicapped Certificate as per brochure format. (If applicable)
• Undertaking regarding registration in the voter list.
• For In-service candidates, NOC from the concerned authority.
• Disability Certificate, if any.
